The University of Chicago Medical Center (UCMC) plans to open a Level 1 Adult Trauma Center in May of 2018.

To tackle the social determinants of health at its new Trauma Center, the UCMC is exploring partnerships with community-based organizations that can provide continuity of care after a patient is discharged.

A key goal of the program is partnership building with local community agencies. Purple Binder is helping the UCMC identify potential partnerships with community services. Purple Binder’s database includes highly detailed, curated descriptions of services offered by community organizations and who is eligible for them. The database makes it easy to find all agencies offering particular types of services, i.e. violence prevention, in a particular neighborhood or zip code.

In addition to identifying potential partnerships, the UCMC will use Purple Binder as a source to identify potential service gaps.
